Just as the skeletal muscles in the body develop under weight, our heart also responds to this load by enlarging its cells in order to overcome the mechanical resistance in front of it. However, this cellular enlargement does not mean that the heart is becoming stronger in a healthy way. On the contrary, the thickened tissue hardens over time, forming the basis of a serious structural deterioration that causes the heart to lose its flexibility, the internal chambers to narrow, and the organ to be unable to fill sufficiently with blood.

How does heart muscle thickening begin and progress in our body?

The working principle of heart tissue is based on an extremely delicate balance. The heart cells of an adult human do not have the ability to divide and multiply when they are damaged or strained. The heart has to increase the volume of its existing cells in order to adapt to the excessive load placed on it. Inside the cells are microscopic building blocks that enable contraction, and these building blocks are arranged in different ways depending on the type of problem the heart is exposed to. If the heart is trying to eject blood against very high pressure, these building blocks begin to be added parallel to each other.

As a result of this parallel addition, the walls of the heart thicken inward. When viewed from the outside, there may not be a huge change in the total size of the heart, but the real problem begins in the internal chambers of the heart. As the walls thicken, the spaces into which blood fills gradually narrow. Since sufficient blood cannot fill these narrowed chambers, the heart begins to send less blood to the body with each beat. In addition, this excessively thickened muscle wall is very stiff; it cannot stretch and open comfortably like a rubber band. The loss of the ability to stretch is one of the first steps on the path to heart failure.

What is the connection between high blood pressure and heart muscle thickening?

The left ventricle of the heart is the main engine that pumps clean blood to the entire body. The greatest resistance this engine encounters while working is the blood pressure inside the vessels. In high blood pressure disease, the vessels are constantly narrowed or hardened. The heart has to expend much more power every second in order to push blood into this narrow and resistant pipe system. Like trying to force open a closed door, the left ventricle also increases its own muscle mass in order to overcome this pressure.

Unfortunately, this growth process does not provide healthy development. While the muscle mass reaches enormous dimensions, the capillary network responsible for nourishing this muscle cannot multiply at the same rate. The resulting picture is this: there is very large heart tissue that requires a great deal of oxygen, but the vascular system that carries blood to it remains insufficient. Even if there is no blockage in the patient’s main heart vessels, severe chest pains occur because of this nutritional deficiency. Over time, heart muscle cells that cannot be nourished sufficiently begin to be damaged and are replaced by nonfunctional scar tissue.

What is the role of genetic factors in heart muscle thickening?

Not every case of thickening has an external cause such as high blood pressure or vascular resistance behind it. Hypertrophic cardiomyopathy, which is quite common in society, is a completely genetically based problem. Faulty genes inherited from the mother or father disrupt the production process of heart muscle proteins. This condition can appear at any period of life, even in childhood. The most distinctive feature of thickening caused by genetic factors is that it is not symmetrical. Not every part of the heart wall thickens equally; usually, the middle wall separating the right and left chambers of the heart grows excessively.

In the genetic form, the structure of the cells is also completely chaotic. Muscle fibers, which are normally regular and face the same direction, become almost like a tangled knot in this disease. This disorder disrupts the contraction rhythm of the heart and causes electrical short circuits.

People with the following conditions are especially at risk:

Family members

Professional athletes

Young individuals

Those with a history of rhythm disorder

In individuals in this risk group, the course of the disease can be much more unpredictable. Especially in young athletes, the most common cause behind sudden cardiac arrest events that develop during training or competition is this genetic heart muscle thickening. That complex structure at the cellular level suddenly locks the electrical system of the heart during exertion, creating a life-threatening danger.

What are the main symptoms that occur in heart muscle thickening?

In the early stages of the disease, since the heart can tolerate this structural change, people may continue their lives for years without feeling any complaints. However, when the thickening exceeds a certain level and the mechanical functioning of the heart is disrupted, symptoms that make daily life difficult emerge.

The most common complaints in patients are:

Shortness of breath

Chest pain

Fainting

Palpitations

Dizziness

Fatigue

Shortness of breath occurs because the thickened heart cannot stretch and relax sufficiently and therefore cannot receive the blood coming from the lungs. When the heart cannot accept the blood, fluid pools in the lungs, and this causes a feeling of breathlessness during exertion. Chest pain, on the other hand, is the result of the muscle tissue that has reached enormous dimensions being unable to receive the oxygen it needs with the existing blood flow. Fainting or severe dizziness occurs when the blood pumped from the heart cannot reach the brain quickly enough during exertion, or when a dangerous rhythm disorder momentarily lowers blood pressure. The presence of these symptoms indicates that the disease has reached a stage that must be taken seriously.

How does outflow tract obstruction affect those with heart muscle thickening?

The greatest mechanical crisis that occurs in a large portion of genetically transmitted thickening is the closure of the pathway through which blood passes from the heart to the vessels. The thickened asymmetric middle wall narrows the heart’s outflow tunnel so much that the blood reaches a very high speed while being ejected through it. Similar to water flowing with much greater pressure when you squeeze the end of a narrow hose, this condition creates a very dangerous physical effect inside the heart.

The vacuum effect created as blood passes through the outflow tract at very high speed pulls one of the leaflets of the nearby mitral valve inward. This valve leaflet, displaced like a door slamming violently in the wind, completely blocks the already narrowed blood outflow tract. The heart has to fight this enormous obstacle in front of it and generate tremendous internal pressure in order to send blood into the artery. At the same time, because the displaced mitral valve cannot close completely, some of the blood leaks in the wrong direction, that is, backward. This mechanical collapse causes the patient to tire very quickly, become short of breath, and the heart tissue to wear out rapidly.

Which tests and methods are used for the diagnosis of heart muscle thickening?

Thanks to the imaging systems offered by modern medicine, it is possible to detect even the slightest thickening inside the heart at the cellular level. The main purpose in the diagnostic process is to measure the size of the wall thickness, identify obstruction in blood flow, and detect rhythm problems.

The main diagnostic methods used are:

Echocardiography

Electrocardiography

Holter

MRI

Echocardiography, in other words heart ultrasound, is the most basic diagnostic tool. With sound waves, an instant map of the heart is created and wall thicknesses are measured millimetrically. In addition, the speed of blood flow and the valve leakage status are clearly seen with color Doppler. Electrocardiography creates the electrical map of the heart and reflects the electrical overload caused by thickening onto paper. A rhythm Holter device is a recording system that the patient carries for a day and is used to detect dangerous momentary rhythm disorders. Cardiac MRI is the gold standard of diagnosis. By showing scars called fibrosis inside the tissue very clearly, it allows risk analysis of rhythm problems the patient may encounter in the future.

How do heart valve problems trigger the formation of heart muscle thickening?

The valve located at the entrance of the aorta, the largest vessel emerging from the heart, can wear out over time and narrow due to calcification. Under normal conditions, this valve opens fully with each contraction of the heart and allows blood to exit freely. However, when narrowing due to calcification occurs, the leaflets of the valve stick together and the exit opening for blood gradually becomes smaller.

The heart, which has to pump blood to the entire body through this very narrow opening, makes an extraordinary effort to resist this great resistance in front of it. Just as the muscles of a person who constantly tries to lift a heavy load become more prominent, the heart walls also thicken because of forcing this closed valve every day. Initially, this thickening is a defense mechanism necessary for the heart to do its job. However, when valve stenosis is not treated, the thickened heart muscle begins to tire. When symptoms appear, the valve must be replaced before permanent damage occurs in the heart tissue. When the valve is replaced and the pathway is opened, the heart muscle relaxes and tends to regress to its former normal dimensions within months.

Is medication sufficient to stop the process of heart muscle thickening?

The first step in the treatment of the disease is always the use of medications that will reduce the stress load on the heart. The medications used generally aim to slow the heart rate, reduce the contraction force to some extent, and increase the relaxation ability of the heart muscle. A heart that works more slowly and more calmly reduces that severe vacuum effect in the outflow tract, prevents the valve from slamming, and relieves chest pain.

However, medication does not eliminate a physical and structural disorder. If the heart muscle has grown excessively and has completely blocked the tunnel almost like a piece of rock, medications become ineffective after a certain point. If the patient’s exercise capacity cannot be increased, fainting attacks cannot be prevented, and daily quality of life has seriously decreased, this is an indication that the limits of medication have been reached. Since it is not possible to melt and eliminate that mechanical narrowing inside the heart with medication, in such cases the treatment strategy is shifted directly to surgical methods involving mechanical and physical interventions.

Which surgical methods are applied in the treatment of heart muscle thickening?

In outflow tract obstructions where medication is insufficient, the permanent solution is surgery in which the source of the problem is directly intervened. The basic philosophy of these operations is to physically eliminate the narrowing inside the heart and return blood flow to its natural course.

The main surgical procedures applied are:

Myectomy

Repair

Replacement

Annuloplasty

The most commonly performed method is septal myectomy. In this surgery, the inside of the heart is directly accessed, and the thickened excess muscle tissue that obstructs the passage of blood is carefully shaved and removed. It can be thought of as scraping open the inside of a blocked tunnel. Thanks to this piece removed from within the heart tissue, the resistance in front of the blood is eliminated and the disease improves dramatically. During the myectomy procedure, repair is often also applied to the mitral valve, whose structure has been damaged by repeated impact. The patient’s own valve tissue is repaired and supported with various suture techniques. Thus, the tunnel is opened and the backward leakage of blood through the valve is definitively prevented.

How are minimally invasive and robotic methods applied in heart muscle thickening surgeries?

In past years, in order to reach the heart, the bone in the exact middle of the chest had to be cut from end to end. However, thanks to developing technology, in patients with suitable body structure, these surgeries can be successfully performed through very small incisions without touching the breastbone at all. The inside of the heart is reached with special surgical instruments and cameras through small windows opened between the ribs on the right side of the chest.

Especially robotic surgery technology offers extraordinary precision in removing that thickened muscle tissue inside the heart. The surgeon sits at the console and sees the inside of the heart in high resolution and three dimensions on huge screens. Even at narrow angles where the human hand cannot enter, robotic arms can rotate in every direction without tremor and cut and remove the excess muscle mass millimetrically. Thanks to these methods, since the bone is not cut, the patient’s postoperative bleeding risk is greatly reduced, the danger of infection decreases, and the hospital discharge period becomes short enough to be expressed in days.

What preparations should be made before heart muscle thickening surgery?

A successful recovery process is directly related not only to the surgeon’s manual skill but also to how ready the patient is for surgery. From the moment the operation decision is made, all organ systems of the body must be strengthened against this major stress. Smoking must definitely be stopped to clean the lungs, and alcohol consumption must be completely discontinued. Prescription medications that affect the clotting time of the blood should be stopped under doctor supervision or switched to suitable alternatives.

Supplements that must definitely be stopped before surgery are:

Ginseng

Turmeric

Ginger

Garlic

The common feature of these natural and herbal supplements, which are known to be very healthy in society, is that they seriously thin the blood. They may cause hidden bleeding that the surgeon may have difficulty controlling on the operating table or afterward. Therefore, consumption of such herbal teas, capsules, and supplements should be stopped at least two weeks before the operation. In addition, how nutrition will be stopped hours before surgery is explained to the patient in detail by the anesthesia team. Compliance with these rules is of vital importance for the surgery to proceed safely.

How should wound care and lying position at home be after heart muscle thickening surgery?

After the intensive and sensitive process in the hospital is overcome and the patient returns home, a new rehabilitation period begins. If the operation was performed by cutting the breastbone, it takes approximately two months for this bone to fuse together again in a healthy way. Just like a broken leg remaining immobile in a cast while waiting to heal, the breastbone should not be strained from the outside.

In order to protect bone integrity, during this two-month period, the patient must absolutely lie only on their back. Lying face down or turning to the sides causes the bone ends to rub against each other, separate, and disrupt the fusion process. When standing up, laughing, or coughing, the chest corset provided at the hospital must definitely be used. To protect against wound site infections, the area should never be scrubbed while showering, and warm soapy water should be allowed to flow gently over the wound. If increasing redness, heat increase, unbearable pain, or discharge is detected in the suture area, it is quite critical to apply to the hospital without losing time.

What should the nutrition plan be like to accelerate recovery after heart muscle thickening surgery?

After heart surgeries, the body can repair itself and the immune system can remain strong only with a proper nutrition program. The body, recovering from the stress of surgery, needs quality building blocks to close wounds and repair the cut tissues. In this period, the Mediterranean-type nutrition model, which does not tire the heart, does not cause vascular stiffness, but supports cellular repair, should be taken as the basis.

Foods recommended for consumption are:

Fish

Chicken

Turkey

Olive oil

Walnut

Almond

Oat

Lentil

Bean

The things that should be avoided are:

Salt

Pickles

Canned food

Sucuk

Sausage

Margarine

High salt consumption causes the body to retain water unnecessarily, develop edema, and blood pressure to rise suddenly. An increase in blood pressure is an undesirable source of stress for a newly operated heart. Especially canned products and pickles are hidden salt stores. In addition, the medications used after surgery may slow down the digestive system and cause constipation. Straining on the toilet applies very high internal pressure to the chest cage bones and may disrupt the bone structure. To prevent this, high-fiber foods such as oats, lentils, and beans should be consumed with plenty of water.

Heart muscle thickening, medically known as myocardial hypertrophy, is the abnormal growth and increase in volume of the muscle tissue that forms the walls of the heart due to the heart being strained under high pressure or genetic abnormalities.
